Overview

Learning to manage diabetes is the best way to prevent or delay its damaging, long-term complications. We are located at the Centre for Life, 140 Durham St. Sudbury. Our hours of operation are 8:30 am - 4:30 pm. We accept referrals from physicians, other health care providers and self-referral. The Diabetes Education and Care Program offers programs and services to people living with diabetes. We are a program of the Sudbury Regional Hospital and the Northern Diabetes Health Network.

Who is a Diabetes Educator?

Diabetes Educators specialize in the management of all types of diabetes. They teach people with diabetes how to eat healthy, be more physically active, when and how to test their blood sugar, and much much more. A Diabetes Educator is someone who teaches, listens and cares about clients with diabetes. Diabetes Educators include specially trained dietitians, nurses, and social workers who are devoted to helping people live well with diabetes.

Diabetes Education gives the client the knowledge and the skills to take control of diabetes.

Adult Programs

Group Classes:

Living Well with Diabetes 1 and 2
This 2-class series will help the person with diabetes learn how to manage diabetes successfully. Topics include healthy eating, blood sugar testing, medications and many others.

A Touch of Sugar
Designed for people with pre-diabetes, also called Impaired Glucose Tolerance or Impaired Fasting Glucose.

How to Win at Living with Diabetes
Discover tips to stay on track, community and financial resources, guidelines for driving, travel and much more.

Introduction to Insulin
An introduction and overall review of insulin therapy including healthy eating tips for those on insulin.

Starting on Insulin
Designed for those who are brand new to insulin or those who could benefit from a review.

Managing on Insulin
Designed to help people tackle the day to day challenges of insulin therapy.

Adjusting Insulin
Designed to teach people with diabetses how to adjust their own insulin.. when they eat more or less, when they are more or less active, and in times of illness.

Carbohydrate Counting for Beginners
Designed to help beginners learn about this exciting healthy eating approach.

Advanced Carbohydrate Counting
Designed to help people learn more advanced carbohydrate counting skills.

Healthy Hearts
Designed to help people learn how to take the best possible care of their heart.

Preventing Diabetes Complications
An opportunity for people to gather important information about how to prevent diabetes complications and to discover what they should be asking their doctor.

Taking Care of Your Feet
An opportunity to learn the do's and don'ts of proper foot care and have their feet examined and blood pressure checked. This program DOES NOT include toenail cutting or other footcare.


Outpatient Insulin Start-up Program
Provides the necessary education, skills training and support for those clients starting insulin. A physician referral is required.

Drop In Day Program
Each Tuesday from 1:30 pm - 3:00 pm, clients are invited to attend drop in. This is designed for clients with more urgent questions or concerns relating to diabetes. Clients may see a nurse, dietitian, and/or social worker for a brief, 15 minute visit. Clients are seen on a first-come, first-serve basis and no appointment is necessary.

Individual Counselling
Individual counselling is available by appointment.


Other Services

First Step Walking Program
The First Step Program is designed to help people increase their physical activity with the use of a pedometer. This program allows the participant to set individual goals with the help of our program leader. It involves four 1 hour group meetings, individual progress reports, and a planning session to personalize goal setting. Each participant is required to purchase a "tool kit" at a cost of $35. Subsidy is available to those who qualify.

Insulin Pump Therapy
Insulin pump therapy or Continuous Subcutaneous Insulin Infusion (CSII) is an alternative to traditional insulin therapy. Patients wear an insulin pump similar in size to a pager. This small device is programmed to deliver insulin 24 hrs a day in small amounts. Regular blood sugar testing is required in addition to nutrition management.

Diabetes Presentations
Our team of diabetes educators present to other health care providers, organizations and community agencies on diabetes-related topics. This service is in high demand and requests are considered on an individual basis.

Endocrinologist
Dr. Boji Varghese, an endocrinologist (diabetes specialist) conduct clinics on-site 2 days per week. A physician referral is required.


Children and Adolescent Program (0-19)

The Diabetes Education and Care Program offers a specialized approach to meet the needs of families with children / adolescents who have diabetes.

These families have access to a wide variety of services including: 

  • In-patient education programs
  • Education on the management of diabetes and related lifestyle issues
  • Assessment of home management and coping strategies
  • Support for the family, child and adolescent through visits, telephone contact and advocacy
  • Clinics include paeditrician, nurse, dietitian and social worker 
  • Crisis management / intervention

The Children and Adolescent Program will help the family plan for school trips, a teenage party, sleeping-in and much more. Families living outside of the Sudbury Manitoulin area also have access to these unique services through various outreach programs.


The Diabetes and Pregnancy Program

This program is intended for women with gestational diabetes or women who have diabetes and become pregnant. It assists clients in managing their diabetes to help ensure a healthy pregnancy and baby. Staff are also available for pre-pregnancy counselling as clients are encouraged to establish healthy blood sugars before pregnancy.
